During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An announced inspection was initiated and completed on 09/24/2025 with a local agency, in response to a complaint received by the licensing office on 09/05/2025 relating to allegations of physical abuse.
The inspector reviewed 2 children?s records and 2 staff records on-site. This inspection included ? document review and tour of the facility.
The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within 5 business days from today, which will be the close of business on 11/12/2025. A POC submitted after this date will not appear on the public website.

It was reported that Adult #1 and Adult #2 were living in the home for about four and three months respectively. Sworn statements were not completed for them.
It was reported that Adult #1 and Adult #2 were living in the home for about four and three months respectively. Fingerprints were not completed for them.
It was reported that Adult #1 and Adult #2 were living in the home for about four and three months respectively. Central Registry searches were not completed for them.
Staff became aware of a reportable offense, and did not report it to the local department immediately.
Caregiver #2, who has been caring for children for one year and a half, could not demonstrate having enough fluency in the English language to maintain a conversation with Licensing representatives.
It was reported that Adult #1 and Adult #2 were living in the home for about four and three months respectively. Tuberculosis screenings were not completed for them.
A room that was not disclosed to Licensing as part of the daycare, was being used for a child to take a nap.
The Provider did not disclose to Licensing about three household members that would be living in the home.
On 09/08/2025, the Provider provided information to a Licensing representative that was later confirmed to be not truthful.
On 09/05/2025, an altercation occurred in the fdh between the Provider and her assistant, in which they exchanged insults while yelling in front of the children in care.
It was reported that on some occasions, Child #2 (20 months), was put to sleep on a bean bag or diapering mat that did not have a bottom cover. Child #2 did have a blanket.
It was reported that, on some occasions, Child #2 (20 months old) was placed to sleep on a stuffed bean bag.
On 09/08/2025, an Arlington County representative observed Child # 1 sleeping in a bouncy chair that is not manufactured for this purpose.
On 09/08/2025, an Arlington County representative observed Ms Azizi on the main level of the home while four children were taking their naps downstairs in the care area. She did not have a baby monitor with her to supervise the children.
