During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site code compliance inspection was conducted on 10/14/2025. The on-site inspection began at 8:58 am and ended at 11:02 am. The inspector reviewed compliance in the areas listed above. There were 12 children present with 4 staff. The inspector reviewed 5
children?s records and 5 staff records on site. This inspection included document review, a tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 10/22/2025. A POC submitted after this date will not appear on the public website.
Violations
4On 10/14/2025, Staff #6 supervised a group of five children ages five months to two years for approximately 25 minutes before an additional staff member arrived. The ratio when infants are present in a grouping of children is 1:4. Staff #6 acknowledged being one child over ratio.
Staff #2, who had been employed for four months, did not have a completed national criminal background check on file.
Staff #3, employed for more than one month, did not have documentation of requesting a central registry check and sex offender name check from one state. The staff member was supervising a group of children independently at the time of the inspection.
Upon enrollment, the center shall require information from the person enrolling the child regarding previous child day care and schools attended by the child.
The records of Child #1, enrolled for over five months, Child #2, enrolled for over two weeks, Child #3 and Child #4, both enrolled for over two months, and Child #5, enrolled for over one month, did not contain information regarding previous child day care and schools attended by the children.
