An unannounced inspection was initiated on 6/12/2025 and completed on 6/18/2025 in response to a complaint received by the licensing office on 6/6/2025 relating to allegations of 22.1-289.031. The inspector reviewed one child's record onsite. This inspection included document review, tour of the facility and interviews.
The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

The center's room with ages zero to 16 months was found to operate with a ratio of one staff member to five children on the day in question.
