Inspector Notes
An unannounced, on-site monitoring inspection was initiated on and completed on 8/13/2025. The on-site inspection began at 8:00 am and ended at 9:30 am. The inspector reviewed compliance in the areas listed above. There were 24 children present and 10 staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements.

The vendor shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions. This list shall be dated and kept confidential in each room or area where children are present.

In the two year old bathroom, bleach and a toilet bowl cleaner were not stored in a locked place preventing access by children.
