Inspector Notes

An unannounced monitoring inspection was conducted on 10/17/23 from 10:40amto 11:40am At the time of the inspection there were 5 children in care and one caregiver. Children were observed eating lunch, participating in floor play and preparing for nap. No medications are administered by the provider. The Inspector reviewed 5 children?s records and 3 caregiver and household member records. Required postings, emergency drills and emergency supplies were also reviewed. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and interview, it was determined the provider did not ensure that documentation of a physical examination by or under the direction of a physician prior to a child's attendance is obtained prior to the child/s attendance or within 30 days after the first day of attendance.
Evidence:
1. The record provided for child #2 indicated an attendance start date of 9/5/2023. Documentation of a physical examination was not in the child's file.
2. The provider verified that she did not have documentation of the child's physical examination.
Plan of Correction: The provider responded, the parent will be notified today that a copy of the physical examination must be submitted.
