Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 1/14/2025. The on-site inspection began at 12:45pm and ended at 2:00pm. The inspector reviewed compliance in the areas listed above to include standards found out of compliance during the previous inspection. There were 7 children present and 3 caregivers; the point total was 19. The inspector reviewed 5 children?s records and 7 caregiver and household member records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements.Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The family day home shall obtain documentation of additional immunizations for a child who is not exempt from the immunization requirements once every six months for children under the age of two years. Child #4 did not have immunization updates on file. The update was due 2 months ago. Child #5 did not have immunization updates on file. The update was due 3 months ago. Both children were under the age of two years at the time of the inspection.
Plan of Correction: The provider has contacted the parents and requested the updated immunization records.
