Parents shall be provided in writing the name of the assistant or substitute provider and the amount of time per week the assistant or substitute, instead of the provider, is scheduled to care for the child. The records for Child #1, #2, #3, and #4 did not contain the names of the current assistant staff and the hours they provide care instead of the provider.
