Inspector Notes

An unannounced renewal inspection was conducted on 11/27/2023 from 10:04 am to 11:36 am. There were 20 children in care, ages eight months to four years, in three groups under the care of three staff. The Licensing Inspector reviewed four children?s records, three staff records, and other program records. The children were observed playing indoors, quiet activities indoors, washing hands, and having lunch.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the facility failed to obtain a copy of the physical examination for a child within 30 days of a child?s attendance.

EVIDENCE:
Child 5 began attendance 4/14/2023 and the physical was done 5/23/2023.
Plan of Correction: From now on, the director will review files to make sure physicals are provided within 30 days of a child beginning attendance.
Standard 8VAC20-780-150-B
Based on record review, the center failed to ensure that a physical examination form for a child was signed by a physician, physician?s designee, or official of a local health department.

EVIDENCE:
The physical exam form for child 4 was dated 4/27/2023 and was not signed by physician, physician?s designee, or official of a local health department.
Plan of Correction: From now on, the director will review the physical forms to ensure they have been signed by the physician.
