Inspector Notes
An unannounced on-site monitoring inspection was initiated on 12/9/2024 and completed on 12/9/2024. The on-site inspection began at 11:28 am and ended at 1:14 pm. The inspector reviewed compliance in the areas listed above. There were 22 children and four staff present. The inspector reviewed three children?s records and four staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 22.1-289.035-A
All employees shall undergo a background check every five years. The most recent Virginia criminal background check for Staff 2 was approximately one month overdue.
Plan of Correction: The Director will obtain an appointment for the VA Criminal Background check today and will review files to ensure others will not become overdue in the future.
Standard 8VAC20-780-350-B-1
The staff-to-children ratio is required whenever children are in care. The ratio for birth up to 16 months is one staff for every four children. There was one staff member and six children in the baby room.
Plan of Correction: A staff person was sick, which cause the out of ratio condition. The Director is interviewing people this weekend to hire two additional staff to prevent this from happening in the future.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the following information: documentation of child updates and confirmation of up-to-date information in the child's record [annual review of parent information and emergency contacts]. The record for Child 1 was last updated 12/5/2021 and the record for Child 3 was updated 12/14/2022.
Plan of Correction: The Director will obtain parent updates of addresses, phone numbers, and emergency information when the parents come to pick up the children.
