Each building that was built before 2015 and that houses a child day program that is licensed and serves preschool-age children shall be equipped with at least one carbon monoxide detector.

The center has 2 buildings that did not have carbon monoxide detectors installed.

Each child shall have a physical examination by or under the direction of a physician before the child's attendance or within 30 days of enrollment.

Child #1 who has been enrolled for over a year did not have documentation of a physical examination in the child's record.

Each staff member shall submit at the time of employment and completed within 30 days of employment, documentation of a negative tuberculosis screening.

Staff #3 employed for 5 months did not complete their TB skin test/screening within 30 days before employment.

Staff shall complete annually a minimum of 16 hours of training appropriate to the age of children in care.

Staff #1 employed for over 1 year did not complete 16 hours of annual training for 2025.
Staff #4 employed for over 2 years did not complete 16 hours of annual training for 2025.
Staff #5 employed for over 2 years did not complete 16 hours of annual training for 2025.

The center shall develop and implement a written policy and procedure that describes how the center will ensure that each group of children receives care by consistent staff or team of staff members.

Center has not developed continuity of care policy and procedures.

The sponsor shall maintain public liability insurance for bodily injury for each center site with a minimum limit of at least $500,000 each occurrence and with a minimum limit of $500,000 aggregate.

The center did not have current liability insurance. The center's liability insurance expired 7/15/2025.

The center shall develop written procedures for prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness.

Staff who work directly with children shall in addition to preservice and orientation training annually complete the department's health and safety update course.
