During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
This inspection notice was amended on June 2, 2025. The Licensee has been provided with a copy of the updated inspection notice.
An unannounced, on-site monitoring inspection was initiated on May 1, 2025, and completed on May 15, 2025, as a part of the licensure period. The on-site inspection began at 3:15 pm and ended at 5:00 pm. The inspector reviewed compliance in the areas listed above. There were 43 children present and four staff. The inspector reviewed five children?s records and five staff records electronically on May 6, 2025. Additional documentation was obtained May 15, 2025. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on June 6, 2025. A POC submitted after this date will not appear on the public website.
Violations
7The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan. Child #12, with a diagnosed food allergy, was not included on the allergy, sensitivity, and dietary restriction list.
At least one staff in each classroom or area where children are present shall have current certification in cardiopulmonary resuscitation (CPR) as appropriate to the age of the children in care. 1. Staff #3, observed alone and in charge of a group of children during the inspection on 05/01/2025, did not have documentation of current CPR certification. 2. Staff #5 observed alone and in charge of a group of children had CPR certification that expired eight months ago. There was no documentation of current CPR certification.
At least one staff in each classroom or area where children are present shall have current certification in first aid from an organization such as the American Red Cross, American Heart Association, American Safety and Health Institute, or National Safety Council. 1. Staff #3, observed alone and in charge of a group of children on 05/01/2025, did not have documentation of current first aid certification. 2. Staff #5 observed alone and in charge of a group of children on 05/01/2025, had first aid certification that expired eight months ago. There was no documentation of current first aid certification.
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include all required information.
1. The injury report for child #5, dated 04/07/2025, was missing the time, date and how the parent was notified of the injury. 2. Injury reports for child #9 (dated 03/17/2025), child #14 (dated 03/13/2025), and child #15 (dated 03/04/2025) were missing staff and parent signatures or two staff signatures. Each reinjury report had one staff signature.
