During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on June 29, 2025 and completed on June 25, 2025. The on-site inspection began at 10:30 am and ended at 11:30 am. The inspector reviewed compliance in the areas listed above. There were 12 children present and 2 caregivers; the point total was 31. The inspector reviewed 5 children?s records and 3 caregiver/household member(s) records [on-site].
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on August 7, 2025. A POC submitted after this date will not appear on the public website.
Violations
5All caregivers must submit a sworn statement or affirmation prior to employment.
Caregiver 2's has worked for over two weeks without completing a statement.
Caregiver 2's record did not contain the name and contact information for a person to contact in an emergency.
Child 2's record does not contain an authorization for emergency medical treatment.
Child 5's records does not list addresses for the two emergency contacts.
Within seven business days of the child's first day of attendance at the family day home, the provider shall obtain from the parent verification of the child's age and identity.
Child 2's record did not contain verification that the child's age and identity had been verified by the provider.
One bottle was observed without a label containing the date or the child's name.
