Inspector Notes

An unannounced renewal inspection was conducted on July 10 2024 between 12:15pm and 1:00pm. There were 5 children (12 points) in the care of the provider. One staff record and two child records were reviewed. The attendance, menu, first aid kit, radio, and flashlight was reviewed and inspected. Citations are listed on the violation notice. Based on record review, the provider did not ensure that all assistants, by the end of their first week of assuming job responsibilities, were oriented.

Evidence: Staff #1 did not have documentation on record that documented an orientation received by the end of the first week of employment.

Based on record review, the provider did not ensure that each child record contained al of the required information.

Evidence: Child #1 record did not contain documentation of two emergency contacts including address and telephone numbers.
Plan of Correction: I have added all the contact information for all the kids.
