The provider shall obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.

The provider did not obtain additional Tuberculosis Screenings every two years from the date of the initial screening for all caregivers and adult household members. Staff #1 is 2 months overdue, Household Member #1 is 2 months overdue, Household Member #2 is 3 months overdue, and Household Member #3 is 2 months overdue.
