Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 9/5/2024 and completed on 9/5/2024, as a part of the licensure period. The on-site inspection began at 10:00AM and ended at 11:03AM. The inspector reviewed compliance in the areas listed above. There were 17 children present and 9 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included ? document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Each report shall include the date of the physical examination and dates immunizations were received and shall be signed by a physician, his designee, or an official of the local health department. Child #2 did not have an immunization record signed by a physician during the inspection on 9/5/24.
Plan of Correction: Provider will obtain signed immunization record.
Standard 8VAC20-780-570-E
Prepared infant formula shall be refrigerated, dated and labeled with the child?s name. Licensing inspector observed 2 bottles of formula that were not refrigerated and labeled with a child?s name during the inspection on 9/5/24.
