Inspector Notes

A renewal inspection was conducted on 2/29/24 from 10:30 am to 12:30 pm. A self reported incident on 2/26/24, related to supervision of children was also investigated during this inspection. There were 40 children present, with 8 staff supervising. The children were observed engaged in small group activities including magna tiles, sensory activities and outdoor play. Name to face count of children was observed prior to exiting the building for the playground. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 children's records, 3 staff records, medications, emergency drill records and injury reports were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only. Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website. Standard 8VAC20-780-160-A
Based on records review, the center did not obtain results of a Tuberculosis (TB) screening within 30 days of the date of employment.
Evidence:
1. Staff # 1's first day of employment was 12/4/23.
2. Staff # 1's TB screening was dated 10/27/23.
Plan of Correction: Managment will ensure new hires complete their TB tests no more than 30 days prior to start date.
Standard 8VAC20-780-340-F
Based on staff interviews and observation, children under 10 years of age were not always within actual sight and sound supervision of staff.
Evidence:
1. On 2/26/24, at approximately 9:30 am, Child # 1 (11 months old) crawled out of the infant classroom into the adjacent Toddler room.
2. Staff # 2 stated that on 2/26/24, Staff # 3 and Staff # 4 were present with a total of 6 infants. Staff # 3 was preparing milk bottles and Staff # 4 was sitting on the floor with the children.
2. Staff # 2 stated that at approximately 9:30 am, she was passing by the Toddler room and noticed the door between the two classrooms opening from the hallway.
3. When Staff # 2 entered the Toddler room, she noticed that Child # 1 had pushed the door open and crawled approximately two feet into the classroom. The door handle did not engage completely, allowing the door to open by pushing.
4. Staff # 2 picked up Child # 1 and took Child # 1 back to the infant room to Staff # 3 and Staff # 4.
Plan of Correction: Retraining the teachers on face to name checks and sight and sound supervision.
Management will continue to monitor the classrooms and face to name checks with staff.
Standard 8VAC20-780-560-G
Based on observation, food brought from home was not dated and labeled.
Evidence: Food containers from home in the 3's classroom were not labeled with each child's name and date.
Plan of Correction: Retrained the staff on proper labeling and food storage procedures .
