Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.
1. In the Infant A room, the cabinet under the diaper changing table was unlocked. Bottles with cleaning solution were stored in that cabinet.
2. In Room 2B, a bottle with bleach solution was left on the counter after tables were cleaned. The bottle was accessible to children.
Plan of Correction: The cabinet was immediately locked. The bottle was promptly removed and stored appropriately. Staff were reminded of proper storage procedures to ensure substances remain inaccessible to children at all times.

Electrical outlets shall have protective covers that are of a size that cannot be swallowed by children.
In Room 3B, 5 electrical outlets on a surge protector strip that was plugged in did not have protective covers.
Plan of Correction: The surge protector was removed and all other outlets are tamper resistant receptacles.

The diapering surface shall be non absorbent.
The diaper changing pad in Room 2B had a tear in the center of the pad.
Plan of Correction: The changing pad was removed and replaced.

A sippy cup with milk in Room 2B was found to be without the name of the child and the date.
