Inspector Notes

An unannounced on-site renewal inspection was conducted on 03/14/2024. There were 28 children, ages 3 months-4 years, and 7 staff members present during the inspection. The inspector observed the following: free choice activities, clean up from lunch and nap. A total of 5 children?s records and 8 staff records, were reviewed. The inspector discussed the following with staff: using the new orientation form found on the DOE website. The inspector arrived at 12:45 PM and departed at 2:30 PM.

Repeat Violation

Based on review of eight children?s records,
the center failed to ensure that all records were complete per the standard.

Evidence:

1.The record for Child # 1 had documentation of the first day of attendance of 02/26/2024 was missing, allergies and intolerance to mediation or any other substances, and actions to take in an emergency situation.
2. The record for Child # 3 had documentation of the first day of attendance of 02/20/2024 was missing chronic physical problems and pertinent developmental information and any special accommodation needed.
Plan of Correction: The director will have the parent to complete the missing items and will monitor all records for completeness.
Standard 8VAC20-780-60-A-8
Based on review of children?s records, the
center failed to ensure that a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence:

1.Child # 1 has a diagnosed food allergy and did not have a written care plan from a physician in the record.
Plan of Correction: The director will have the parent to obtain the allergy action plan ASAP.
