During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 06/09/2025 and completed on 06/14/2025. The on-site inspection began at 11:00 AM and ended at 1:00 PM. The inspector reviewed compliance in the areas listed above. There were 43 children present and 14 staff. The inspector reviewed 5 children?s records, 13 staff on-site. This inspection included document review, tour of the facility and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 06/20/2025. A POC submitted after this date will not appear on the public website.

Program directors shall be at least 21 years of age and shall meet one of the requirements for education and experience.

The staff # 5 did not have the educational documentation in their record.

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

A container of sanitizing hand wipes with warning labels, ?Keep out of reach of children,? were unlocked in every classroom and accessible to the children.

The center shall implement a monthly practice evacuation drill.
