Standard 8VAC20-780-160-A
SYSTEMIC VIOLATION

Each staff member is required to submit documentation of a negative tuberculosis screening, which must be completed within the last 30 calendar days prior to the date of employment and signed by a physician.

Staff #5 had a TB result dated three months prior to the date of employment.
Plan of Correction: Reviewed policy will be sure all TB test are accurate and up to date per the standard before start date.

The center shall develop and implement a written policy and procedure that describes how the center will ensure that each group of children receives care by consistent staff or team of staff members.

At the time of inspection, the center did not have a written consistent care policy and procedure.

SYSTEMIC VIOLATION

The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies. This list shall be dated and kept confidential in each room or area where children are present.

Classrooms serving children with documented allergies had allergy lists posted; however, the lists were not dated.

Children?s hands are required to be washed with soap and running water after toileting and after any contact with blood, feces, or urine, and staff are required to wash their hands with soap and running water before and after each diaper change.

Staff in Classroom #2 and Classroom #3 did not wash children?s hands after diaper
changes. Staff in Classroom #3 did not wash their hands before changing a child?s diaper.
Plan of Correction: Communicated to all staff about proper handwashing after toileting to align with the standards guidelines.

For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Classroom #5 had 15 children present, but only 12 were signed in on the attendance. Classroom #6 had 12 children present, but 15 were signed in.
Plan of Correction: Will be sure that classroom sign ins are correct and each child is signed in to the correct classroom they are in when moved for ratio purposes.
