Inspector Notes
An unannounced monitoring inspection was conducted today between 10:30am and 11:50am. There were 11 children present with the Provider and 2 assistants. The children were observed participating in circle time, eating a morning snack (banana slices, apples and mandarin oranges) while watching cartoons and playing freely with blocks and cars. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

11Evidence: Caregiver #3s record does not contain fingerprint results. The provider confirmed that caregiver #3 has not yet completed fingerprinting.
Evidence: Caregiver #3s record does not contain a completed sworn disclosure statement. The provider confirmed that caregiver #3 has not signed a sworn disclosure statement.
Evidence: Caregiver #3s record does not contain documentation of two references, contact information of someone to be contacted in case of an emergency, documentation of education, background checks as required by 8VAC20-770 and tuberculosis results as required by 8VAC20-800-170.
Evidence: The provider confirmed that caregiver #3 has not provided a TB screening. Caregiver #3 was observed working during todays inspection.
Evidence: Child #9s record does not contain addresses for both emergency contacts.
Evidence: There were two small tears observed in the diaper changing mat.
Evidence: An EpiPen was observed in the pocket of a backpack on the bottom cubby of the childrens storage cubbies. The EpiPen was not locked.
Evidence: Two diaper creams were observed in the diaper changing area without names to identity the owners.
Evidence:
1. A and D cream was observed that expired 3/2019.
2. Desitin cream was observed that expired 6/2021.
Evidence: There is no evacuation drill documented for the month of September. The provider stated that they did a drill but didn't document it.
Evidence: There is no menu posted.
