Inspector Notes
An unannounced renewal inspection was conducted today between 11:10am and 12:30pm. There were 10 children present with the Provider and 1 assistant for a total of 23 points within the supervision guidelines. The children were observed watching television while finishing a morning snack, washing hands and playing with building blocks. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Evidence: Caregiver #3s record does not contain documentation of orientation. The provider stated that training was completed, but they could not produce documentation that was signed and dated.
