Inspector Notes

An unannounced renewal inspection was conducted today between 11:10am and 12:30pm. There were 10 children present with the Provider and 1 assistant for a total of 23 points within the supervision guidelines. The children were observed watching television while finishing a morning snack, washing hands and playing with building blocks. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Based on record review, assistant records did not contain all required information.
Evidence: Caregiver #3s record contained one reference check.
Plan of Correction: We did it and it should be in the record. It must be in the office. We will put it in the record.
Standard 8VAC20-800-200-C
Based on record review and interview, documentation of orientation training was not maintained.
Evidence: Caregiver #3s record does not contain documentation of orientation. The provider stated that training was completed, but they could not produce documentation that was signed and dated.
Plan of Correction: We did it and the form should be in the record. It must be in the office. We will put it in the record.
Standard 8VAC20-800-60-B
Based on record review, childrens records did not contain all required information.
Evidence: Child #2s record does not contain the families medical insurance policy number.
Plan of Correction: We've been asking the family to provide this. The parent was contacted during the inspection.
Standard 8VAC20-800-90-C
Based on record review, the family day home did not obtain documentation of additional immunizations every six months for each child age two and under.
Evidence: Child #23 (20 months) most recent immunizations are dated 2/3/2023.
Plan of Correction: We will get it from the family.
