Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 7/18/2024. The on-site inspection began at 1pm and ended at 1:40pm. The inspector reviewed compliance in the areas listed above. There were 11 children present and 2staff. The inspector reviewed 2 children?s records and 1 staff record. This inspection included document review (i.e. Injury logs, policies and procedures, emergency drill logs, authorization forms, a tour of the family day home, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on record review and interview, the family day home did not obtain out-of-state background checks for each employee that has resided outside of the state of Virginia within the last five years.
Evidence: Caregiver #3's sworn disclosure statement documents that the caregiver has resided in North Carolina within the last five years. The provider confirmed that they had not completed a central registry background check or sex offender background check for North Carolina for caregiver #3.
Plan of Correction: We will get them completed as soon as possible.
