During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced , on-site monitoring inspection was initiated on 12/02/2024 and completed on 12/02/2024. The on-site inspection began at 11:00 am and ended at 12:55 pm. The inspector reviewed compliance in the areas listed above. There were 7 children present and 3 caregivers; the point total was 22. The inspector reviewed 8 children's records and 4 caregiver/household member records on-site.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
6The provider shall maintain an up-to-date record at the family day home for each enrolled child.
The enrollment records did not contain all of the required elements as noted below:
a. A second emergency contact and medical insurance policy number for child 1.
b. A phone number and address for the second emergency contact for child 2.
c. A work phone number for one parent, second emergency contact and parent signature for completion of enrollment form for child 3.
Before the child's first day of attendance, parents shall be provided in writing the following information:
The written parent agreement lacked the following required information as noted below:
a. Amount of time per week that an adult assistant is scheduled to care for the child and the name of the adult assistant was not indicated in the record of child 2,4,5,6 and 7.
b. Policies for administration of medication for child 2, 7 and 8.
c. The written medication policy in the file of child 3 conflicts with current medication policy as indicted by the provider.
Written documentation of updated immunizations were not on file for child 2 , 5 and 7. These children were in care during the inspection.
The menu for the current week beginning 12/2/2024 was not posted.
Bottles for child 1 and child 2 were not dated.
Infant cereal had been added to the bottle of child 7. The provider fed this bottle of thickened formula to child 7 while in care on the morning of the inspection.
