Inspector Notes
An unannounced renewal inspection was conducted on 2/13/2024. The inspector was on site from approximately 9:05 am-11:00 am. There were 24 children present, ranging in ages from 4 to 6 years, with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies, nutrition and background checks. A total of 5 child records and 6 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.
Violations
3Evidence:
1. An injury report for child #1 was reviewed. The record indicated that child #1 sustained a minor injury on 12/5/2023. The document indicated that the parent was notified on 12/6/2023. The document contained three signatures that were dated 12/6/2023.
2. Administration acknowledged that the recorded date of notification was not the same day as the date of occurrence.
Evidence:
1. The center's menu listed a morning snack of pancakes and juice. Two classrooms were observed eating snack. The snack offered to the children consisted of only pancakes. During interview, the staff acknowledged that the juice was not served. Snacks are required to contain two of the five components which are fluid milk, meat/meat alternatives, vegetables, fruit, and grain. The morning snack offered to the children only contained one component. (grain)
2. Administration acknowledged that the snack did not meet the requirements.
Evidence:
1. The record of staff #4 (date of employment: 3/20/2023) did not contain documentation to demonstrate that the individual possesses the education required by the job position. Staff #4 was identified as a lead who qualified through a combination of experience, training and education. The education component was not in the record.
2. Administration acknowledged that the staff was qualified but they were missing the proof of education.
