Inspector Notes

This non-mandated, follow-up monitoring inspection was initiated by licensing staff and concluded on 11/13/2023, between the hours of 1:30 pm to 2:30 pm. There was 1 toddler-age child present (3 points), with 1 supervising. The child was engaged in the following: naptime, diaper changing, hand washing, snack, and other organized activities. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication (Provider obtain MAT certification); special care and emergencies; and nutrition. A total of 1 child's records, 1 staff record, and 3 household records were reviewed.

Information gathered during the inspection determined that there were (2) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the home.

Based on review, 1 child's file indicated that not all of the required documentation was contain within their file.

Evidence -
1. On the date of inspection (11/13/2023), the following documentation was not available for review for Child #1: the Medication Administration - Decision to Administer form; and the Provisions of the Emergency Preparedness and Response Plan form.
2. The start date for Child #1 was listed as - 10/30/2023.
Plan of Correction: Both missing documents will be completed and placed on file.

Based on observation, the emergency numbers were not posted.

Evidence -
1. On the date of inspection (11/13/2023) the following numbers were not posted: 911, and the number for the regional poison control center.
2. This was confirmed on the date of inspection by the Provider.
