Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 12/12/2024. The on-site inspection began at 10:15am and ended at 12:15pm. The inspector reviewed compliance in the areas listed above to include standards found out of compliance during the previous inspection. There were 5 children present and 2 caregivers; the point total was 14. The inspector reviewed 5 children?s records and 2 caregiver records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 1/7/2024. A POC submitted after this date will not appear on the public website.

The license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises. The most recent inspection findings dated 8/8/2024 were not posted.

Repeat Violation
The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. The record for Child #1 and Child #2, enrolled for more than nine months, did not contain documentation of a physical examination.

Standard 8VAC20-800-240-A
Areas and furnishings of the family day home, inside and outside, shall be
maintained in a clean, safe, and operable condition. There was cracked, loose and jagged mirror inlay pieces on the sides of a hall table. The table was in the entryway to the childcare classroom.

Standard 8VAC20-800-320-B
Child-resistant protective covers larger than 1-1/4 inches in diameter shall be installed on all unused electrical outlets and surge protectors accessible to children under five years of age. The outlet in the bathroom did not have a protective outlet cover. The electrical power strip in the childcare classroom did not have protective outlet covers.

Hot water at taps available to children shall be maintained within a range of 105?F to 120?F. The water temperature of the faucet children used to wash hands was 125?F when measured with a thermometer.

Standard 8VAC20-800-430-A
Repeat Violation
Children under two years of age and children over two years of age who are not developmentally ready to climb or descend stairs without supervision shall not have access to stairs. There was no gate in use to prevent access to he staircase. Children had access to the staircase when using the bathroom, washing hands and arriving and departing the home.
Plan of Correction: The provider responded that she has put the gate on the wall in front of the tree and stairs and the children will not be able to access the stairs.

Shelter-in-place procedures shall be practiced a minimum of twice per year. There was no documentation of shelter-in-place drills practiced within the past 12 months.

When food is brought from home the container shall be clearly labeled in a way that identifies the owner. A bottle with a white fluid in it was sitting on a table in the childcare classroom. The bottle did not have a name identifying the owner.

Before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations. The records for Child #1 and Child #2 did not contain documentation that the children had adequate immunizations.
