During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 3/13/25 and completed on 3/17/25. The on-site inspection began at 10:00amand ended at 12:00pm. The inspector reviewed compliance in the areas listed above to include standards found out of compliance during the previous inspection. There were 4 children present and 1 caregiver; the point total was12. The inspector reviewed 4 children's records and 2 caregiver records on-site. Additional records were viewed electronically on 3/17/25. This inspection included document review, tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3/31/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-320-E
Portable electric heaters, and similar heating devices located in areas accessible to children shall have barriers or screens and be located at least three feet from combustible materials. A portable heater was in the childcare room. The heater was accessible to children and did not have a barrier or screen.
Plan of Correction: The heater will have a barrier placed between the children and the heater when in use.

Repeat Violation
The family day home shall obtain documentation of additional immunizations for a child once every six months for children under the age of two years. There was no documentation of additional immunizations for Child #2. Updated documentation was due 2 weeks ago.
Plan of Correction: Provider has given parent a 30 day notice if documents are not turned in. Appointment date is 3/22/25.
