Inspector Notes
An announced initial inspection was completed on 08/23/2023 from 9:35am until 11:25 by the inspector and the licensing administrator as a result of a planned change in ownership. Required postings, physical plant including the playground were inspected. Applicant records had been reviewed at the office.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation, the center did not maintain the center is a safe condition.
Evidence:
1. On the playground, there was an exposed screw approximately one inch long between two pieces of wood being used as a barriers for resilient surfacing.
2. The fence at the back of the playground had two places where wood missing at the bottom that are splinter hazards.
3. A toilet set that the children use is worn and wood is showing.
Evidence:

Plan of Correction: The director stated that the fence and nail would be fixed on the playground and that the toilet seat would be replaced.
