Inspector Notes
An unannounced inspection was initiated and completed on 5/7/2024 from 10:50am until 11:50. as a follow up inspection in regard to an intensive plan of correction submitted.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision.
Information gathered during the inspection determined non-compliance with applicable standards or law and violation (s) were documented on the violation notice issued to the program.

Based on record review, the center failed to have in each grouping of children at least one staff member who meets the qualifications of a program leader or program director shall be regularly present.
Evidence:
A classroom had a teacher who did not have the documentation that meets lead teacher qualifications.

Based on observation, the center failed to meet the ratio requirement in a classroom.
Evidence:
1. In a classroom there were 7 children and one teacher with the youngest being 1 yr. 11 months. The ratio requirement is 1 to 4.
