During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced on-site inspection was initiated and completed on 8/14/25. The inspection began at noon and ended at 1:50. The inspector reviewed compliance in the areas listed above. There were 17 children and 6 staff present. The inspector reviewed 5 children?s records and 4 staff records. This inspection included document review, tour of the facility, interviews, and observation.

Information gathered during the inspection determined non-compliance with the applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-260-A
The center shall provide to the licensing representative an annual fire inspection.

The last fire inspection was dated 06/2024.
Plan of Correction: The fire official had been called prior to the inspection and is scheduled to complete the inspection in August.
Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials shall be kept on a locked place using a safe locking method that prevents access by children.
Cleaning products were being stored on a shelf behind a curtain in the school-age bathroom.
Plan of Correction: The hazardous substances will be moved to a locked location.
Standard 8VAC20-780-330-B
When playground equipment is provided, resilient surfacing is required in the fall zone of climbing equipment.

The mulch was approximately 3 inches thick in the fall zone of the monkey bars.
Plan of Correction: Mulch will be spread to the required depth.
Standard 8VAC20-780-40-M
The center is required to keep a current list of all know allergies, sensitivities and dietary restriction in each room or area where children are present and it must be kept dated and confidential.
The list in the infant rom was posted on the refrigerator with no cover sheet and the list in the kitchen was not up to date.
Plan of Correction: The list was covered and the list will be updated.

One working, battery-operated radio is required at each center.

The radio at the center was not working and or did not have the correct batteries.
Plan of Correction: A new radio and batteries were bought.
