Inspector Notes

An announced initial inspection was conducted on 8/25/23. At the time of entrance, 2 children totaling 4 points were in care with the provider. The record sample size consisted of 4 children's records and the provider's record and the household memeber's record. The children in care were observed engaging with the provider, eating snack, handwashing, participating in educational activities and free play. There have been no injuries and no medications are being administered at this time. Areas of noncompliance are noted in the violation narrative and were discussed with the provider during the exit meeting at the conclusion of the inspection.

Standard 8VAC20-800-100-A
Based on record review and interview it was determined the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. The records for child #2, #3, and #4 did not contain documentation of a physical exam.
2. Provider confirmed that this documentation was not available during this inspection.
Plan of Correction: Provider will ask parents to provide this documentation.
Standard 8VAC20-800-240-A
Based on observation and interview it was determined the provider did not ensure that all areas and furnishings of the family day home, inside and outside, were maintained in a clean, safe, and operable condition.
Evidence:
1. The two shelving units in the childcare areas rocked when pulled on presenting potential tipping hazards.
2. Provider confirmed these units were not anchored.
Plan of Correction: Provider will anchor the shelving units.
Standard 8VAC20-800-60-B
Based on record review and interview it was determined the provider did not ensure that the record for each child contained all of the required information.
Evidence:
1. The record for child #1 did not contain their medical insurance policy number.
2. The record for child #2 did not contain addresses for the two required emergency contacts, medical insurance policy number, and written authorization for emergency medical care.
3. The record for child #3 did not contain addresses for the two required emergency contacts, medical insurance policy number, and written authorization for emergency medical care.
4. The record for child #4 did not contain their medical insurance policy number.
5. The provider confirmed that these children are enrolled and this information was not available during this inspection.
