Inspector Notes
An unannounced monitoring inspection was conducted on 10/31/23. At the time of entrance, 4 children totaling 16 points were in care with the substitute provider. The record sample size consisted of 4 children's records and the provider's record and the substitute's record. The children in care were observed engaging with the provider and napping. There have been no injuries since the last inspection and no medications are being administered at this time. Areas of noncompliance are noted in the violation narrative and were discussed with the provider during the exit meeting at the conclusion of the inspection.

1. There were no references or documentation of orientation in the record for the substitute provider(hired 10/11/23).
2. Substitute provider was alone with the children in care throughout this inspection.
