An unannounced, on-site monitoring inspection was completed on 07/22/2025, as a part of the
licensure period. The on-site inspection began at 12:45 pm and ended at 1:40 pm. The inspector
reviewed compliance in the areas listed above. The provider is transitioning from offering day-
time to evening and overnight care. There were no children present and 1 staff. The inspector
reviewed 1staff. This inspection included a document review, tour of facility, interviews,
observations and measurements. The information gathered during the inspection determined no
violations with applicable standards or law. No violations were issued.
