During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated and completed on 3/10/26 in response to a complaint received by the licensing office on 3/4/26 relating to allegations regarding safe sleep and daily activities for infants. This inspection included interviews and observations. The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 3/23/26. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-790-730-G
Cribs are required to have at least 12 inches of space between them when occupied.

On 3/10/26 occupied cribs were observed to be placed immediately next to one another where the open slat sides were touching.
Plan of Correction: Staff were retrained on proper spacing between cribs/cots when occupied.
Standard 8VAC20-790-730-J
Cribs are required to be placed where objects outside the crib such as electrical cords are not within reach of infants or toddlers.

On 3/10/26 the electrical cord to the refrigerator was observed to be plugged in, hanging directly behind the crib of an infant, within reach of the infant when they stood up in the crib.
Plan of Correction: We repositioned the cribs to not be in reach of any cords.

Children?s hands are required to be washed with soap and running water or disposable wipes before and after eating meals or snacks.

On 3/10/26 at approximately 10:15am, the infant class was observed playing on the playground then transitioning inside. Six infants were immediately given bottles inside without their hands being washed before or after drinking the bottles.
Plan of Correction: Staff were retrained on proper handwashing practices & times to do so.

Staff are required to wash their hands with soap and running water after any contact with body fluids.

On 3/10/26 staff were observed wiping infant?s noses with tissue then not washing their hands on four occasions.

When a child is placed in an infant seat, high chair, or feeding table, the protective belt is required to be fastened securely.

On 3/10/26 eight feeding table chairs were observed to have had their buckles removed. The bottom of the seats were more than 24 inches off the ground. Eight children between the ages of 7-months to 16-months were observed in the feeding table without buckles to secure them.
Plan of Correction: All infant table seats had seat belts reinstalled & staff retrained to secure infants when sitting at table.

Bottles are not to be used while the child is in their designated sleeping location.

On 3/10/26, staff were observed giving bottles to three children while in their designated sleeping location.
Plan of Correction: Staff retrained on safe sleep practices & not giving bottles to children in a sleeping location.
