An unannounced, on-site monitoring inspection was initiated on 09/12/2024 and completed on 09/18/2024. The on-site inspection began at 9:40 am and ended at 10:40 am. The inspector reviewed compliance in the areas listed above. There were 17 children present and 2 staff. The inspector reviewed 5 children?s records onsite and 2 staff records electronically on 09/18/2024. This inspection included document review, tour of the facility, interviews, and observations.

REPEAT VIOLATION
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Child #1, who had been attending for approximately two weeks, did not have documentation of an immunization record on file.
Plan of Correction: Staff will contact admin staff to obtain.
