Inspector Notes

A non-mandated monitoring inspection was conducted on February 20, 2024 from 1:30 P.M.-2:45 P.M. The inspection was a focused inspection regarding a serious injury report that was received by the Office of Child Care Health & Safety (Valley) on February 12, 2024. The inspector reviewed compliance in Staff Qualifications and training, staffing and supervision, special care provisions and emergencies and special services. One child record and two staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-580-I
Based on interview, the center failed to ensure that before leaving on a field trip a schedule of the trips events and locations shall be posted and visible at the center.

Evidence:
1. On 02/09/2024 Staff #1 and Staff #1 took approximately 13 children (ages 3/4/) on a walking field trip to the local playground. They did not post a trip schedule at the center of the walking field trip.
2. Administration #1 stated on 02/20/2024 that they have never posted a field trip schedule when classrooms are out on walking field trips.
Plan of Correction: A sign will be posted on the door for all field trips when taken.

Based on documentation review and interview, the facility failed to secure parental permission for field trips before the scheduled activity.

Evidence:
1. On 02/09/2024, Staff #1 and Staff #2 took approximately 13 children (ages 3/4), for a walking field trip to the local playground.
2. There was no documentation of parent permission for the walking field trips on file for Child #1.
3. Administration verified that written permission for walking field trips had not been obtained.
Plan of Correction: Permission forms for walking across the road to the playground have been obtained for all children. Future enrollees will complete form upon enrollment.
