Inspector Notes

A complaint inspection was conducted on May 8, 2024 from 1:10 P.M.-3:00 P.M. A complaint was received in the Office of Child Care Health & Safety (Valley) on May 2, 2024 regarding behavior guidance, supervision, and injury reports. The inspector interviewed three staff, reviewed three staff records, two children records, and seven injury/incident reports.

The information gathered does not support the allegation, therefore the complaint is determined to be "not valid". There was information gathered during the inspection, not complaint related, that determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-530-A-1
Based on observation and review of staff records, the center failed to ensure that at least one staff in each classroom or area or where children are present shall have a current certification in cardiopulmonary resuscitation (CPR).

Evidence:
1. Staff #3 's record did not contain documentation of current certification in CPR. Staff #3 was observed working alone in a classroom with a group of children.
2. Staff #3 verified that her CPR certification was not current.
Plan of Correction: In the future we will ensure that all staff members are CPR certified within 30 days of employment.
Standard 8VAC20-780-530-A-2
Based on observation and review of staff records, the center failed to ensure that at least one staff in each classroom or area or where children are present shall have a current certification in first aid.

Evidence:
1. Staff #3's record did not contain documentation of current certification in first aid. Staff #3 was observed working alone in a classroom with a group of children.
2. Staff #3 verified that her first aid certification was not current.
Plan of Correction: Center will ensure all staff members are certified before being left along in a classroom.

REPEAT VIOLATION
Based on a review of staff records, the center failed to ensure all required information was in the staff records.

Evidence:
1. Staff #1's date of hire was 04/09/2024. Staff #1's record only contained one reference as to character and reputation as well as competency were checked prior to employment..
2. Staff #2's (date of hire 04/18/2024), and Staff #3's (date of hire 04/08/202), records did not contain documentation of two references as to character and reputation as well as competency were checked prior to employment.
3. Administration verified that the records was missing references for Staff #1, #2 and #3.
Plan of Correction: All information was on computer but could not be printed off at time due to copier issues. Staff has ensured that as soon as printer works all items needed will be printed off.
