Inspector Notes

An unannounced inspection was initiated on 08/13/2024 and completed on 08/16/2024 in response to a complaint received by the licensing office on 08/01/2024 relating to allegations of staff qualifications and training and staffing and supervision. The inspector reviewed 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 08/23/2024. A POC submitted after this date will not appear on the public website.

Based on review of records and interview, the center failed to ensure that in each grouping of children at least one staff member who meets the qualifications of a program leader or program director is regularly present.
Evidence:
1. Staff #3 and Staff #7 were observed to be the only staff in the two-year-old room. Neither Staff #3 or Staff #7 met the qualification of program leader or program director.
2. Staff #4 and Staff #8 were observed to be the only staff in the school age room. Neither Staff #3 or Staff #7 met the qualification of program leader or program director.
3. Staff #1 confirmed that Staff #3, #4, #7 and #8 did not meet the qualification of program leader or program director and that there was no other staff the met the qualification of program leader or program director regularly present in either classroom.
