An unannounced inspection was initiated on 09/03/2024 and completed on 09/03/2024 in response to a complaint received by the licensing office on 08/22/2024 relating to allegations of Administration, Staff Qualifications and Training, Physical Plant, Staffing and Supervision, Special Care Provisions and Emergencies. This inspection included document review, tour of the facility, interviews and observations.

The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
