During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on March 6, 2025 and completed on March 7, 2025. The on-site inspection began at 9:50 a.m. and ended at 12:35 p.m. The inspector reviewed compliance in the areas listed above. There were 17 children present and four staff. The inspector reviewed five children?s records and nine staff records on-site. This inspection included document review, tour of the facility, interviews and observation.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on March 17, 2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-240-B
Staff shall complete orientation training in section C of this section prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.
Staff #5 completed orientation training three months late.
Plan of Correction: Will ensure staff members are getting orientation in a timely manner.
Standard 8VAC20-780-260-B
After the first license, annual approval from the health department shall be provided.
The program?s most recent health inspection was completed six months late.
Plan of Correction: Will contact the Health Department close to expiration to ensure someone is coming.

Repeat Violation
Areas and equipment shall be maintained in a clean, safe and operable manner.
There were several pieces of trash on the playground including a can with sharp edges and jagged plastic pieces from broken toys.
There was standing water on the sand table lid, in two play kitchen sinks, and in two riding toys.
Plan of Correction: A staff member has been assigned with checking the outdoor area daily.

When centers choose to provide meals or snacks, the following shall apply: a menu listing food to be served for meals and snacks during the current one-week period shall be posted in a location conspicuous to parent or given to parents.
The menu posted was dated for October of 2024.
Plan of Correction: Menus will be made and posted on a monthly status.
