During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site inspection was initiated on June 10, 2025 completed on June 16, 2025 in response to a complaint received by the licensing office on June 3, 2025 relating to allegations of Administration, Programs and Special Care Provisions and Emergencies. The inspector reviewed one children?s records. This inspection included document review and interviews.

The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to Programs and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on June 24, 2025. A POC submitted after this date will not appear on the public website.

The following actions or threats of thereof are forbidden: Striking a child.
Child #1 threated to punch Staff #2 and Staff #2 told Child #1 that they would clock out and whip Child #1?s behind.
Plan of Correction: Staff #2 is a family member of Child #1 and was told by family to make the threat. Staff was told to never make threats of forbidden actions regardless of the request to do so and will be attending training on upcoming new discipline policy.
