Standard 8VAC20-780-140-A
Each child shall have a physical examination by or under the direction of a physician before the child?s attendance or within 30 days after the first day of attendance.
Child #2, who had been attending for more than three months, did not have documentation of a physical exam on file.

Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening.
Staff #4?s updated screening was due three months ago and had not yet been completed.

Staff shall complete a minimum of 16 hours of training annually, appropriate to the age of children in care.

The carpet in the two-year-old classroom was frayed on two of the four sides leaving loose strings.

Staff #6 was observed supervising six children (youngest child 23 months old) from approximately 10:00 a.m. till nap time (around 12:30 p.m.).

The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present.
The allergy list in the two-year-old classroom was ten months old and did not have one child?s food sensitivity listed.
The allergy list in the infant room was ten months old and did not have one child?s dietary restriction listed.
There was no list in the PreK room and there were two children with allergies, sensitivities and/or dietary restrictions.
Plan of Correction: List was corrected and posted in each room with the date.

Medication shall be labeled with the child?s name, the name of the medication, the dosage amount, and the time or times to be give.
There was one epinephrine pen being kept in the office for a child. The epinephrine pen was not labeled with the child?s name, dosage amount or the time to be given.
Plan of Correction: We do not give medication. Was sent home with the child.

REPEAT VIOLATION
When centers choose to provide meals or snacks, the following shall apply:
A menu listing of food to be served for meals and snacks during the current one-week period shall be dated and shall be posted in a location conspicuous to parents or given to parents.
The menu posted was dated for the previous week. The menu for the current week was not posted.
Plan of Correction: Menu was printed and posted and menus are now made on a monthly basis.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.
The record for Child #1, enrolled for about three months, was missing the following required components: first day of attendance and proof of identity.
The record for Child #3, enrolled for about one month, was missing the following required components: first day of attendance and work number and place of employment for one parent.
The record for Child #5, enrolled for eleven months, was missing the following required components: name, address and home phone number of one parent, and work number and place of employment for one parent.
Plan of Correction: All information was corrected on each form and parents were contacted to correct information.
