Inspector Notes

A monitoring inspection was initiated on 12/8/2023 and concluded on 12/8/2023. There were 26 children present with seven staff supervising and two office staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and eight staff records were reviewed. The inspection started at 10:00 am and concluded at 1:15 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22.1-289.035-B-2
Based on record reviews the center failed to ensure that all staff have a fingerprint based criminal history check prior to the first day of employment.
Evidence:
Staff #6, 7 (hire date 9/12/23) do not have a fingerprint based criminal background check on file. Both staff were working during this inspection.
Plan of Correction: These are former staff that came over when the business owner was changed. Will ensure that all background checks are completed within the time frames.
Standard 8VAC20-770-60-B
Based on record reviews the center failed to ensure that all staff have a Sworn Statement completed prior to the first day of employment.
Evidence:
Staff #1 (hire date 11/15/23), Staff #3,4,5,6,7 (hire date 9/12/23) do not have a sworn statement on file.
Plan of Correction: Corrected during inspection. Director's will ensure that all required paperwork is in the staff files by the required time frames.

Based on record reviews the center failed to ensure that all staff have a Central Registry check on file by the end of the 30th day of employment.
Evidence:
Staff # 6,7,8 (hire date 9/12/23) do not have a central registry check on file. All three staff were working on the day of the inspection.
Plan of Correction: These are former staff that came over when the business owner was changed. Will ensure that all background checks are completed within the time frames.

Based on observations the center failed to ensure that the areas of the center shall be maintained in a safe condition.
Evidence:
In the infant room, the area used for the younger infants had an extension cord surge protector that was hanging down next to the window.
