Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 8/27/2024 and completed on 8/27/2024. The on-site inspection began at 9:00 am and ended at 11:00 am. The inspector reviewed compliance in the areas listed above. There were 24 children present and six staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, observation and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on record review, the center failed to obtain a copy of the results of a sex offender registry check from any state in which the individual has resided in the preceding five years.

Evidence:
Staff #1, Staff #3, Staff #4 and Staff #5 did not have a sex offender registry check from any state the individual has resided in the preceding five years in their staff record.
Plan of Correction: The Director completed the registry checks on 8/27/2024.
