Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated on and completed on 08/26/2025. The inspection started at 11:00am and concluded at 12:30pm. There were four children present and one caregiver; the point total was 14. The inspector reviewed four children?s records and two caregiver/household member records on-site.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Standard 8VAC20-790-190-B
Vendors shall maintain, and keep at the family day home, written or electronic information for each enrolled child, which shall be made available to the department's representative upon request. The child's information shall include the name, address, and telephone number for each custodial parent's place of employment or school attendance, if applicable. The records for Child #1 and Child #2 did not include the address and phone number for the parent?s place of employment.
Plan of Correction: I will collect the missing information and have it added to the children's records.
Standard 8VAC20-790-210-B
Caregivers shall undergo tuberculosis (TB) screenings at least every two years from the date of the initial screening, or more frequently if recommended by a physician. The record for Caregiver #1 contained a TB screening dated 07/20/2023. An additional TB screening was overdue by five weeks.
Plan of Correction: I will schedule an updated TB screening.
Standard 8VAC20-790-250-F
Caregivers who work directly with children shall, in addition to preservice and orientation training required in subsections A through D of this section, annually attend at least 16 hours of training, to include the department's health and safety update course. This training shall be related to child safety, child development, health and safety in the family day home environment, and any required department sponsored training. Caregiver #1 stated three hours of annual training was completed during the previous year.
Plan of Correction: I will complete the required training hours by the end of the month.
