Inspector Notes

An unannounced monitoring inspection was started in the center on February 27, 2024 from 4:10 PM until 5:55 PM and concluded on March 4, 2024. At the time of entrance 23 children were in care with 5 staff members. The sample size consisted of 5 children?s records and 5 staff
records. Children were observed involved in table top activities, eating and playing in centers. The playground was not inspected due to inclement weather.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Repeat
Based on observation and staff interviews, the licensee did not ensure. Areas and equipment
of the center, inside and outside, shall be maintained in a clean, safe and operable
condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or
otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or
warping, rusting or breakage of any equipment; head entrapment hazards; and protruding
nails, bolts or other components that could entangle clothing or snag skin.

Evidence: The center's kitchen door was observed unlocked and open; the door is in the
center?s main hallway and accessible to children. In the kitchen, there was an unlocked
draw with cooking utensils and a knife. The knife poses a hazard to children.

Based on observation, the licensee did not ensure hazardous substances and other
harmful agents such as cleaning materials, insecticides, and pesticides shall be kept in a
locked place using a safe locking method that prevents access by children.

Evidence: The following items were observed and were determined to be hazardous substances.
1. In the unlocked kitchen, on the counter there were 2 containers of bleach/ water solution.
2. In the 2?s and 3?s room on a shelf was a spray bottle of bleach and water solution.
Plan of Correction: utilize the childproof lock for the kitchen door, and keep the other bottles always locked up in the classroom

Based on record review and staff interviews,the licensee did not ensure staff records
contained all required documentation.

Evidence: References for Staff #3 were not available for review. Center Director confirmed
Staff #3 was working.
