Inspector Notes

An unannounced inspection was initiated on 1-16-2025 and completed on 3-12-2025 with a local agencies, in response to a complaint received by the licensing office on 1-16-2025 relating to allegations of Staffing /Supervision and Programs. The inspector reviewed 1 child's records and 4 staff records on-site. This inspection included document review, interviews and observations.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violation is documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 3-19-2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-400-A
Center did not ensure that in order to promote a child's physical, intellectual, emotional, and social well-being and growth, staff shall interact with the child and one another to provide needed help, comfort, support. Video footage shows that on January 9, 2025 in Room #1, Staff #1 was observed walking past Child #1 who fell face forward while strapped into a rocker seat. Child #1, with the rocker attached to them crawled over to a group of children who were being fed by Staff #2. Staff #1 did not provide support until approximately 90 seconds later.
Plan of Correction: 1/16/2025: All staff that were present in the classroom during this incident are no longer employed with us.
1/20/2025: All staff undergo extensive training on our positive guidance policy, which provides detailed instructions on properly caring for every child in our program.
1/20/2025: All infant staff and floaters have been trained on what infant support equipment is appropriate for what size/age of infants in our care.
3/18/2025: Infant staff and floaters have been assigned to complete the ?Age Appropriate Activities for Infants and Toddlers? training to provide additional knowledge on the reducing the use of any infant support equipment.
