The center is required to obtain a completed sworn statement prior to the employee's first day of employment. Staff #1 has been employed for more than 3 weeks, and Staff #2 has been employed for more than 3 months and both staff do not have a completed sworn statement.
Plan of Correction: Staff #1 and Staff #2 are no longer employed with A+ Childcare. New Hires will be required to complete and sign a sworn statement prior to first date of employment.

Cleaning and sanitizing materials shall not be located above food and shall be stored in areas physically separate from food. On 7-8-2025, in the kitchen granola bars were observed stored next to bleach and water spray bottles and soap and water spray bottles

The required ratio for children 3 year old up to school age eligible is 1:10. On 7-8-2025 the ratio was not met as follows:
1.Two staff (Staff #7 and Staff #8) were observed supervising 22 children, the youngest age 3. The group was out ratio for approximately 17 minutes in the morning.
2.Two staff (Staff #7 and Staff #9) were observed supervising 23 children, the youngest age 3. The group was out ratio for approximately 26 minutes in the morning.
Plan of Correction: 1.Additional staff has been hired lo ensure the staff is in ratio during peak morning hours.

All required documentation shall be maintained for staff records.
1.Staff #1, Staff #2, Staff #3 and Staff #5 did not have documentation required to be kept at the center regarding any health problems that may interfere with fulfilling the job responsibilities.
2. Staff #2 and Staff #4 have been employed do not have documentation that two or more references as to character and reputation as well as competency were checked before employment.
Plan of Correction: 1. Staff #1 and #2 are no longer employed al A+ Childcare. Staff #3 and #5 have both signed documentation and ii has been placed in their files.

A written agreement between the parent and the center shall be in each child's record by the first day of the child's attendance. The agreements listed below shall be signed by the parent(s).
1. An authorization for emergency medical care should an emergency occur when the parent cannot be located immediately unless the parent states in writing an objection to the provision of such care on religious or other grounds.
2. A statement that the center will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible if so requested by the center.
3. A statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately.
Child #2 (attending for 2 days) did not have agreements 1, 2 and 3 signed in their record, Child #4 (attending for almost 3 months) and Child #5 (attending for over 1 month) did not have agreement 3 signed in their record.
