Inspector Notes
An unannounced monitoring inspection was initiated on 7-1-2025 and completed on 8-12-2025, with a local agency, in response to a self-report received by the licensing office on 6-27-2025 relating to area or part of standard, Staffing and Supervision and Programs. The inspector reviewed 1 child record and 2 staff records on-site and electronically on 7-1-2025 and electronically from 7-9-2025 up to 8-12-2025. This inspection included document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. .
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8-19-
2025. A POC submitted after this date will not appear on the public website.
Violations
1or shaking a child. On 6-26-2025, Staff #1 popped Child #1 with their open hand on the
right part of their head near their eye. Child #1 immediately put their hands up to their face
where they were struck.
2.Held mandatory staff training to ensure each team member is aware and understands positive guidance policy. Staff meeting held 7-18-2025.
